That fish can't win, can it? Have you checked and double checked parameters in quarantine (pH and ammonia come to mind). It seems it is about time for you to start a hyposalinity treatment...

You're probably not going to want to hear this but I've just come across this in "PocketExpert Marine Fishes"
........(Sadly, in recent years some individuals have failed to survive more than a month; the reason for these mortalities is not known, although collection or shipping problems are suspected by some observers.) ........
I know it's not the answer you're looking for but thought it might put your mind at rest if you know it's not something you've done!

Yellow Tangs are the hardiest of the Surgeonfish when it comes to keeping them in captivity, but surgeons such as the Powder Blue and the Regal are the most prone to Ich.
That's why I only have a Yellow Tang, I'm too scared to try the others:grinyes:

I had him for a month or so, not positive when I bought him, but on my little timeline it's been roughly a month. I'm going to hold off replacing him with something until I move.
Blossom, just be careful when choosing fish. If you're a beginner, go for beginner fish. Save ones that are a little bit less hardy for when you have a bit of experience under your wings.
