Are you using any supplements in his diet? This includes, calcium, D3, and vitamins? If so, which ones and how often?

It's a calcium and D3 blend, about 3x a week

That's a lot of D3. D3 should be used once a week, if even that. Calcium should be 2-3 times a week.
Possibilities are MBD or hypercalcemia due to D3 overdose. MBD is more common, and I have had it even though I dusted food, as my gecko wasn't absorbing it correctly.
My best suggestion is going to an exotic vet. It's the best solution to get the answer you need. They often give meds, and liquid calcium.
Vicious_Fish may have some good suggestions as well, he deals with leopard geckos too :)

It's a tough process to deal with as well sometimes. Here's my girl, who is recovering nicely now. She broke her leg, as you can tell in the picture. This just shows what can happen.

She wasn't eating at the time (which is why she was skinny), mostly due to having such a soft jaw. She's looking a 100 times better now, wish I had an updated pic to show how they can improve, but I don't like disturbing her that often!
Goodluck!
