Okay, I am finally trying to sit down and work through the permit application. Not too hard, except for a couple of blocks.

4)b - is there a Classification for mystery snails? I used Pomacea Bridgesii as the scientific name in 4)a.

4)d - if I put a quantity of 100 in here, even though I doubt I will ship 100 in a year to any state, will this be okay?

4)f - Are mystery snails pests that are established in the US?

4)g - what are the major hosts of the "pest"?

7) - I put Newspaper as the host media to be shipped with them. Is this okay?

8) - destination of a state, without a specific city. Is this okay?

9) - port of arrival I have as "Unknown".

10) - approximate date of arrival I have as "Uknown".

12) - what do I put for the supplier?

14) - intended use I put as "Hobby".

15) - what do I put as the method to prevent pest escape?

16) - what do I put for method of final disposition?

After that, I just have to sit down and complete them for every state and figure out how to submit them.

This is almost as much fun as watching mystery snails in the aquarium. NOT

4)d - if I put a quantity of 100 in here, even though I doubt I will ship 100 in a year to any state, will this be okay? You don't need to do quantity

4)f - Are mystery snails pests that are established in the US? No

4)g - what are the major hosts of the "pest"? I left this blank

7) - I put Newspaper as the host media to be shipped with them. Is this okay? Yes, I also put water and polyfill on mine.

8) - destination of a state, without a specific city. Is this okay? Yes

9) - port of arrival I have as "Unknown".

10) - approximate date of arrival I have as "Uknown".

12) - what do I put for the supplier? Self or hobbyists

14) - intended use I put as "Hobby".

15) - what do I put as the method to prevent pest escape? Sealed aquarium/container for shipping

16) - what do I put for method of final disposition? Freezing or crushing

You don't have to fill out the specifics as if you were shipping, just the state from and too.

When I applied for my, after I'd completed about half the applications, I discovered that you can copy a previously submitted application, edit the state, and submit it with the new state. I don't remember for sure, but I think it's an option on the "My applications" page.

Thanks for the replies everybody, especially msjinkzd. I was thinking about going the paper route instead of the online route because Adobe would just allow me to change the state and print, versus re-entering all the info, but if the online method allows just the changing of the state, I'll go that route.
