Danny came into chat for some advice with this tank... 29 gallon shrimp tank also stocked with otos. Says it's been cycled approx. three weeks.
Main concerns now about possible ich treatment options that are safe for both shrimp and otos together. However, also need to confirm whether this is ich first. Danny described it as fuzzy white. When asked if it looks grainy or fuzzy, he said it's grainy and fuzzy.
Suggestions?

Hard to tell from you pics, can you post more?
In the beginning stages they may appear sprinkled tiny tiny white almost bubbly dots or 1 or 2 definitive white dots.
I would ask then does it look like tiny white bubble like dots sprinkled over its body? & in it's finnage are there more pronounced white dots in the veinage of the fin? then I would say yes it's Ich.
Just last week I had a bout with Ich with my discus. One had the bubble sprinkles as well as dots in its finnage, 2 others had 1-2 dots.
Lupin suggested salt treatments 1teaspoon for every 5gal & raise the temps (I raised mine from 82 to 86, yours may need to rise 4-5 degrees if you're in the 70's) after water changes. Lupin also suggests continuing for 14days more.
In the meantime I ordered Kordon Rid Ich (as well as Prime - best prices from www.kensfish.com) & started treatment with salt. (but be careful not to overdose as it contains malachite green & formalin - 2 ingredients that are quite strong).
It took 5-6days - the sprinkles disappeared on one with its finnage showing small white smears instead of dots, like they exploded like how the egg whites look after cracking them open.
but if not as I have described, I would venture a guess some kind of fungus but not sure what to suggest for meds.
others will be able to help you out, but need better pics & better descriptions.
also size of tank, temps, water parameters (best to use API liquid master kit), how often you do water changes & how much at a time, do you use conditioned or aged water, type of fish & quantity, filters, what recent changes have you made that you can remember, etc.
more info the better the feedback you will get from AC'ers.

Looks like ich to me. I just finished curing my tank of it. I raised the temp to 84 and used Kordon Ich Attack twice a day. It took about 3 weeks but everyone is now ich free. It works but you have to have patience. By the way I have shrimp, snails and plecos.
