What is the tank stock? Another fish may have picked on him. Also, what size is your tank and what are the water parameters? Is he showing any other symptoms?

he is the only fish in there water is just fine last time i checked the ph is 7.0

Need ammonia/nitrite/nitrate/and temp too.

Looks like a costia infection to me. Best way is to raise temp to 84 degrees + aeration. You might want to consider a malachite green or coppersafe med treatment as well, but after you confirm that it is a costia infection by comparing to pics online.
