Gourami with injury, maybe HITH?

Last night I noticed my pearl gourami looks like she's missing some scales on the top of her head. There's nothing really sharp in the tank, besides driftwood, and I haven't seen any other fish picking on her. Not sure if it could be HITH, it doesnt *quite* look like the pictures, but it's hard to tell.

She also seems to be hanging out at the top of the tank more than usual. She is eating though, and doesn't seem particularly stressed- still colored up, no clamped fins. I do water changes once a week-every 10 days or so. The tank's been up for almost a year, water parameters are stable, 0 ammonia and tries, trates are always under 10, probably because it's heavily planted. Water is soft. No problems with the tank, except 1 ich outbreak that's been over for a while. Stock list is in my sig. I feed tetramin flakes, frozen brine shrimp and bloodworms.

I'll try to get pics when I'm home. Anything I can do, besides water changes? I've heard thoeries taking carbon out can help HITH, so I can do that and see if it helps.

The same thing happened to my female pearl. I decided the culprit was my male pearl, who had a habit of harassing the female. I think she must have scraped herself on a piece of wood or perhaps my pleco's big rock with holes while trying to get away from the male. As I recall, all I did was to keep up with my water changes (5 gallons twice a week on my 29-gallon tank) and keep an eye on her. She healed up nicely. Be sure to keep up the water changes to ward off the fish gremlins.
