Help with cloudy tank

I have a 20 gallon tank in my home. I had goldfish and a "cleaner fish" but they recently died. I had had them for approximately 2 years and all was well. The only thing that changed was I temporarily put in standard 40 watt lights (2) in the cover since my standard aquarium bulbs had burned out. I only had these lights on for about 2 hours. In hindsight, I can only think this change caused them to die. Is this possible?

Also, my main question is that since this occurred, I totally emptied the tank; cleaned all items including the gravel; refilled the tank with tap water; cleaned and replaced the "whisper" filter"; put appropriate lights in the tank lid and let it sit for 2 days. I also have a "diver" to aerate the tank. Lo and behold, I bought new goldfish (12 small ones) and within a day, 1 died and my tank seems cloudy. What can I do to clear this up. Suggestions?...
