supplemented with kents liquid fertilizer once a week 1/4 teaspoon

my platy and my oto died a day ago, now 2 harlequin rasboras are dead.
this leaves me with 4 harlequins, 1 platy, and 1 oto. Corpses all look fine, no aggression was witnessed. what is going on???
Also, I use a HOB filter and it's been a long time since changing the carbon. I've heard that activated carbon, if given long enough will release all of it's stored toxins into the water? I've also heard that carbon is fine if you use it for a long time because the pores in the carbon hold beneficial bacteria. Could this be the problem?
