Do you heat your water to the same temperature as your tank? Are the water parameters (pH, KH) the same as the ones in the tank?
I suspect it has something to do with temperature though, since it can affect the amount of dissolved oxygen, if I'm not mistaken.
Hope you find the answer sooner than later

Are you using a dechlorinator, like Prime or AmQuel+?

I'd ask the same as Roan, but also ask how large and how frequent your water changes are.

when you do a water change make sure that the water you are adding is not hotter than the water in the tank.....and also are you sure these fish arent already sick and the only time you really check is when you do a water change??
edit: is hotter a word?

How big is your tank?
How many/what kind of fish do you have?
How long have you had the tank up and running and how long have you had your fish?
Answer these in addition to the other questions already posted.
We can't help if we don't know anthing.

the temp may be a bit warmer than the tank water but i read in many books that that would be ok like a a couple of degrees warmer shouldnt hurt the fish.i dont check the KH but i do know the ph is lower than the tank water . tap water 7.0 and the tank water is about 7.3

How often do you do your water changes? Fish are very sensitive to pH (and other stuff) that can build up with infrequent changes.

TetraAqua
AquaSafe the tanks ammonia, nitrite and nitrates readings are all zero
and i change my water 50% at least once a week but the last water change i just did was a 80% and i cleaned the tanks substrate very well and removed a few large rocks and fake plants to give a little more room for easier clean up
