there's a couple of things.

one was, many of the blue rams are coming from Asia and there have been some issues with these fish.
another thing..is there is a question of how long these fish live..on average the result has been 1-4 years.

outside of that you can only look for signs of illness. which is a possibility .

Are his fins clenched and does he have labored breathing? My bolivian ram gave me a similar scare recently. My LFS helped me with it and I bought some PraziPro, takes care of internal parasites and some other stuff, and Yellowbelly (my ram) recovered beautifully from whatever it was. Anyhow, he was lethragic, was pale and listless, breathed heavy and layed on the gravel with his fins clenched and just let the current of the filters push him over. Other fish were picking at him even though he wasn't dead, that's how bad he was. But there were no other external signs of disease.
Good luck,
Mary.

I had problems with m large tank awhile back.

turned out to be internal parasites.

I was able to treat with jungle parasite clear..has metro and prazi in it.
I also picked up some levamisole in case the parasite clear didn't work.

this affected my discus and severla angelfish..but they looked normal except they didn't eat..

my largest angel went almost 3 weeks without eating.

it's difficult to do diagnosis of these issues.

I can't add to the advice you have already received, but I assume your tank water temp is at least 82? and that all your nitrogen issues are ok? (0,0 and less than 10.) That you have been doing regular water changes, etc?
Hope things work out.
Cathy

blue rams are prett sensitive and of the water isn't right they can stress and open up to infestations(like most fish)

check your water parameters
