angel fish

hello i woke up this morning and found ont of my zebra angels up in the corner with one of its fins not moving i have had it for about 3 months now and never encountered this before. Does anyone know whats wrong? It is a 120 g tank and i do 25% water change every 5 days, they get fed flake food in the morning brine shrip at 9 and blood worms at 11. The temp is at 78

what are the water params? (ph, nitrate, nitrite, ammonia?)

when i fist started out i few turned out like that and later passed on, i figure it's either water quality, or some kind of internal parasite

we are going to need your ammoina, nitrite, and nitrate readings along with what else is sin the tank besides the angel to tell you whats going on
