Open soar on the nose/face of 2inch cory cat

Basically a few days ago I noticed a small white spot on the nose of my cory catfish. not as small as ick per say. Almost like a raw spot. I kind of figured that it happened from digging his face into some corner to try to get some food or something but I noticed tonight it looked a little bigger, could it be that he is still burying his face in/under a rock or wood and is just making it worse? Or is this a disease/infection? Something contagious? Something that will go away on its own? Suggestions/Comments. He definitely seems to be doing fine. Always on the hunt.

What kind of substrate do you have? Small grained sand? Small rounded pebbles/gravel? Sharp edged angular gravel?
 
long time ago when i was young a cory i had was looking like its mouth was disintigrating, and was told to remove any sea shells i had in the tank and its mouth started to heal and grow back. maybe if there are shells in there, try taking them out, if not, might have gotten an infection of some sort.

My substrate is smooth gravel heavily planted and lots of drift wood.
Peaceful tank mates. Tank params. fully cycled nitrates 30-40, ph6.4 day 6.0 night. GH medium/hard kh 80-90.

Same thing happened to one of my corys, I just did a water change and added melafix for a week or so, healed right up. Just a scrape.
 
cant tell if its getting better

The cory cat totally gets enough food. he is always searching it out and I make sure to stick the tubiflex chunks to the glass at the bottom so all the fish can get some. It really looks like if you or I had a cut or a scrape and spent the whole day in the pool/ocean. White and raw.
