I had a yellow lab cichlid that had a greyish black tint to its cheek and neck area-- kinda like it was carrying eggs

I assumes it was carrying.. so I thought nothing of it... but today I found it dead...

does this sound like a disease I have never heard of or was it a coincidence?

Nitrate- 15
nitrite- 0
ammonia- 0


just recently switched out to sand.. but he was in there over a week with no issue

Yeah they can show up black faces as they mature. Also when stressed. Perhaps it was getting a bit of a pasting/was sick anyway and the black mask reflected the stress. As it turned up dead I would say maybe not a conicidence - but doesn't seem like your water conditions contributed at all.
