Help! Somethings wrong with my lotus!

About 3 weeks ago my tiger lotus sent up its first lily pad to the top of the tank. Its been at the top for almost 2 weeks now and the lily pad wont open up. It got about halfway opened up and just stopped in its tracks. I don't know if this means anything but I've been finding lotus leaves that have apparently been torn off the mother plant all over the tank. Again this is in that area where all my plants seem to wither away and die. (Extreme form of decay disease, remember thread?) EXCEPT its about a foot away from the original spot. Idk if this helps but the stem does appear to have been 'bent' at an angle and within the past day or 2 the lily pad corners have been torn off. If you believe the bulb is going dormant explain why it has 2 new sprouts at the rhizome within the past week and everything is still growing quite Noticeably.

Do you see any fish picking at it? I have an angelfish who constantly picks on my tiger lotus leaves at the surface.

try to clear away some of that duckweed.
Let the leaf float on the surface undisturbed.
