Goldfish red lump

Today my goldfish was bottom sitting and I see he has what looks like a red pustule with a short white feature in the middle. It looks nasty.

What could this be?spots-spot.jpg

He is in a stable 26 gallon tank and I check him out most days so this must have emerged in no more than a few days.

Red lesions usually indicate a systemic bacterial infection. Could be other organs are affected which would account for the loss of coordination and buoyancy issues. If he's still eating I would try a broad spectrum antibiotic in his feed. Good luck.

Still working on this. Tried some anti-bacterial feed but he is anorexic so not sure how much is getting eaten

BTW water params: ph 7.5, Ammonia 0, Nitrite 0, Nitrate 40. I am thinking of getting a hardness test just in case that is an issue.
