Sick clown loach

I have a clown loach that has started acting very odd. Over the last week I noticed that it was never coming out of a cave so today I moved the cave out of concern. It seems to be very sick but I have no idea why! The other loaches in the tank are fine (4 of them) The water parameters seem fine all fine and the pH is sitting around 6.8. No new fish have been added in over a year. Now it is doing a very awful thing, it is floating agains the intake of a powerhead! Its mouth seems to be rotting or something horrible. As it i my largest loach I am clearly heartbroken and hope there is something I can do.

Sorry to see that! Looks like it could be fungus or (hopefully not) columnaris. I'd quarantine immediately and probably treat for columnaris if I was in your shoes

He is extremely lethargic but still able to swim if he really wants to. He is able to stay upright. He seems very round compared to before and I just noticed that he has a bubble or something sticking out of his underside. I am starting to think it is a blockage!
