My fish comes to the surface to breathe

I must doing something wrong, what can it be?
I am a newbie and don't have much experience.
I have a plastic tank and I fill it 90-95%.
I change almost 50% of the water every 3 or 4 days.
I don't have any stuff inside the tank.
Is it because of the tank or do I use a lot of anti chloride or something?

Filter? Using a good dechlor that works on chlorine and chloramine?
 
What kind of fish?

There are many fish that will gulp at the surface and it's totally normal behavior.
 
What kind of fish are you keeping - and in what kind of container? Just reading what you have posted so far I can't help but think betta bowl, is that correct?
 
Tank size? Stocking? Ammonia, nitrite, nitrate? What dechlorinator are you using? Temperature? What kind of filtration are you using?
