Injury or disease?

One of my lemon tetras on its side near its stomach looks like its missing scales and is torn up a bit. It doesn't appear to be deep and just a deep flesh wound. It looks white and squishy i guess but it is not on top of the scales but the scales are missing which is why i assume injury and not fungus.
I tried for about 15min to get a pic but hes just as active as always so its quite hard. He is eating normally as well.
No new additions in the last month so i don't think its anything from a store.

My water is 0ammonia 0nitrite 5-10nitrate(pending time of the week)
60% water change ever 6 days. 78F.

Whats steps should i take?
If its only an injury is it really worth QTing him? or would that just be more unneeded stress if he seems to be happy now?

sounds to me like an injury which is getting infected. if it were me i'd probably qt him so i could treat it.
 
alright gonna move him, what should i treat with?
 
i'm really not sure what the meds are called, sorry. i usually treat injuries with a little bit of salt/heat....i'll bump this up for you so someone who knows the name of the meds can answer for you.
 
Well hes still active but not really eating, salt and heat has not seemed to do much. The area is starting to get pink in some spots.. don't know what that means.. any suggestions?
