I have a white Oranda with an orange head, and this morning I noticed that there are two noticeably sized white patches on his head. Can any one tell me what they are?

I just found my plecostomous dead on the bottom of my tank. My oranda is just fine, but the plecostomous is literally cut in half. Sorry for the details, but all the organs including eyeballs are gone, all that remains is the tail, and bones.
Yes, it's a Biocube29. I only have 1 of the fluorescent lights in, as I took the other one out. Is the actinic lighting the purple "moon lights"? I used to have those on all night, but haven't since this happened.
By the way, now my water is cloudy/misty/foggy and my ammonia is .25, nitrates...

I had an algae bloom earlier in the week, and I've been struggling with it since. I had it cleared out with good looking water, and earlier today I noticed that my water is getting cloudy again, and I'm not quite sure what to do.

It's not in direct sunlight but I generally kept the floureacent light on all day and switched to the purple uv light at night. The filter before the bio-balls was getting so dirty that water wasn't able to pass through it effeciently and my water pump would run dry. I can keep the light off...
