    You don't need a marine tank to keep puffers. There are plenty of freshwater species (ie fahaka, mbu, dwarf, congo, etc) and bracking species (ie GSP's, figure 8's, etc.).

    I can't help you on the disease/injury ID, but the fish is a Synodontus notata x Synodontis ocellifer hybrid. They're fairly common in the hobby and will usually get to about 8 inches long, 9" being the maximum.
