Found a short article that gave me the basics I think...
ref: http://badmanstropicalfish.com/articles/article15.html

Before I could even get started I will need...
$50 permit to import (lasts 1 year)
$25 permit to use non-designated port of entry (lasts upto 2 years)

Then for each shipment I will need...
$55 for U.S. Fish & Wildlife inspection
+$40 if at a non-designated port of entry
+$9 for customs paperwork

The thing that I have questions about is how to minimize costs and what qualifies as a shipment? (does a "shipment" = 1 box, multiple boxes, cargo container etc)

I'm interested in keeping an eye on this.

I don't know much about this but I know for sure that shipment refers to whatever you receive at one time. A shipment could be 1 fish or 100 boxes. It is just whatever is received at one time

You will need all of the above. In addition you will need to arrange shipment with the airlines to send it in air cargo or if it is just a couple of boxes you could do it Fedex, DHL, or UPS. Freight costs are very high for international shipments so that will easily triple or more your fees for customs and permits.

Lastly you will need to work out method of payment with the exporter. This may involve a wire transfer which also has fees and there may be an exchange rate to deal with if they do not accept payment in US currency.

Sounds like you would do better to get together with someone already importing fish, and make a deal with them to import for you. Since they already have the paperwork, should be a lot easier to bring them in that way, and probably a lot cheaper. Look on Aquabid, there are several importers, some of which would may be willing to help you.
