dead paradise fish diagnosis

My paradise fish went from acting normal to lethargic to dead in 4-5 days, I though she was constipated, but the swelling didnt go away after fasting. Clamped fins, laying on the gravel, no pine-coning, no swelling in a specific location, just looked bloated), Water parameters are fine (though her condition seemed to get worse with each water change. More disconcerting, she has these weird ulcers/lesions near her tail, and discoloration by on her gill (only on her right side), tough to see without bright light.

Any idea what it is?

Sorry about your fish. Hard to tell the cause from the pics. What are your readings in PPM for ammonia, nitrite and nitrate?

Ive only had her 4 weeks, shes been living in the 15g quarantine tank with my comet goldfish. She was fine for weeks, though you could see her not react well to the water changes (hiding in the corner for a day). The symptoms come on after a water changes, I did another 2 days later thinking maybe the water was off somehow. Then she got even worse. I did one more water change before going to get the meds. Near death by the time I got back.

The water itself, and the change schedule was the same as my 50g. No other fish had any reaction. Im more considered about whatever it was cross contaminating and risk to the goldfish. Buckets, scrapers, nets tubing have all been used on both tanks. Even air stones and maybe sponge filters, I wasnt being particularly careful which went into which tank after cleaning.
