HC keeps dying

I doubt too much light as the tank was running with only 2 tubes for about 2 months and HC didn't grow much. After another t5 added I see more new leaves sticking out.. Okey here is the dosing of flourish doseage.jpg I don't dose equilibrium and those stuff at day 7. I dose all the Seachem Fluorish ferts

Simular things seems to be happening with HM and glosso, glosso was looking great, saw some melting spots after trim today
