ick cause??

I have had an aquarium for awhile now. I vaccum the gravel and change 25% of the water twice a week. I consider my tank to be pretty clean and has no algae in it. Recently, my fish have been dying and I have noticed some getting ick and fin rot. I hav started medication, but what causes these things to happen?? Where does ick come from?
 
ich is generally transferred into the tank via a fish or plant.

also ornaments, fish nets, your hands or anything else that can come into contact with another tank that has ich.

A lot of the time, new fish will carry ich in their gills and transmit it to the rest of the tank. Fish that are stressed by bad water conditions, overcrowding or a myriad of other reasons will display ich more readily than others. That does not mean that the others don't have it, just that it is not presenting itself in visible locations.

Treating the tank with the salt + heat method for at least (IMO) 14 days should fully erradicate the ich from the tank.
