Need help with chords

Hello, i know this may be a long shot, but i was hoping to see if there was someone that could help me out on figuring out chords to a favorite song of mine. i cant seem to find an acoustic chord sheet, or any chord sheet for that matter. if there is anyone here who is good at figuring out chords and making chord sheets and stuff id love your help. this is the song: [YT]QEvmQzUljos[/YT]

What is the name of the song? I may have the sheet music to it, with chords and all.

is this better? and its called "If I Die" by something corporate. and ya i just want the chords to it, i dont play guitar, i play ukulele so reading guitar tabs is kinda impossible for me :P but i do want it to sounds pretty close to the video if thats at all possible :)
 
Oh I don't know the chord names for uke. But I know how to play x.x sorry

thanks jake, but unfortunatly i cant read that for uke :/ that guitar tabs you got there, if i had just the chord names than id be fine. but im not really familiar with guitar chords
 
Sorry about that. I feel like a goof now. You could figure it out, but it would be a huge pain... Hmmm...
 
You can find a guitar chords chart, and match up the chords with the tabs, then simply play those chords on your uke
