Why did my Lemon Tetra die? *pic*

I noticed it had a white spot on the top of it's head the day before it died. The others didn't have this. I have no idea what it is, maybe someone knows?

The spot can be seen much better in the water, but this is the best I could do:

Ok, tank specs:

55 gallon:

5 aldult platies
20 platy fry(under 1") When they get 1.5", I will sell them to the LFS
3 adult mollies
2 4" plecos
10 cardinal tetras
4 lemon tetras(now 3)

Nitrate: 20ppm(I'm doing my weekly water change tomorrow)
Nitrite: 0
Ammonia: 0

It has a emperor 400 for filtration. The fish were not bullied at all, they got along real well.
 
How long have you had him for? Could be just old age ,became weak which would leave him vulnerable to disease. Do you know the age of him? If he is not that old then it is definately something else.
 
maybe it was bullied while at the store or before, or was injured in transport (maybe before you even got it)

In the picture, are its scales missing on that spot? or are they just faded?
 
Maybe you just were unlucky and got a bad fish...Its eye didn't look like that before it died did it? Also, depending on how fast the place you bought it from sells out of fish, it might be worth a trip to see how the others from the tank look
 
That looks like a normal Lemon Tetra eye to me..
