I had a lot of plants crash like that also. It was because there is one fish in a 20 high tank and whatever I do I can't get the nitrates up enough to keep the plants healthy. I have the Flourish with nitrogen but even it only helps marginally. I'm just going to have to give up and plant it with plastic. Poor little dwarf puffer is losing all his forest and needs more than just a hollow log for cover even though he is a loner in the tank.
Please keep in mind that this tank has been up and running for 2+ years with yearly major take down, thorough cleaning and also monthly maintenance without any crashes. That's what makes it so mysterious. And, this tank has fish. I have included a picture of this tank taken last year June.

A co2 injected tank with 2.4 watts per gallon cannot go for a month without a water change. All your nutrients have been depleted by then, and fish food and waste will not replace all of them. I notice that you have not listed what your ph or ammonia levels were? or if you were adding any other forms of fertilizer?
