Beta Question

My sister has a beta and after she changed the water she noticed that her hyperactive beta all of a sudden became lethargic. She has two different betas and the water temperature of both tanks were the same. My sister became worried since he was not acting normal and he was sideways. There were bubbles on his fins, which could have prevented him from trying to swim. The fish died about 15 minutes later, what could have caused this?
 
What were the readings on the tank, specifically ammonia?
 
The only thing i can think of
1.Even though she might have thought the water she was adding was the same temp.It could have been a few degrees off putting the fish into shock though i have rarely seen this happen unless there was a significant change in temp.
2.Was the betta eating well?

The beta was eating well.
 
did she use soap when she cleaned the tank, or any other chemicals besides dechlorinator?

She did not use soap, or any other chemicals to clean the tank.
 
my question is, how long between water changes? the fish could have gone into shock due to crappy water becoming happy water, too much and too soon, aka old tank syndrome.

Also, are these recently purchased fish? it may have had some condition prior to coming home and that is what did it in, not the water change, although the water change may have been just enough stress to put it over the edge.
