Ailing Congo Tetra

I'm at a bit of a loss as to what's happening to one of my Congo Tetras.
He has a small, whitish, slimy-looking patch on his forehead. It doesn't look like velvet or columnaris.

Two of the other five congos in the tank spawned today, and a pair of kribensis last week, so I'm a bit skeptical that this is a water quality issue. Ammonia, nitrites, nitrates are fine. pH is around 6.8.

I've added a bit of Melafix to the water and have set up a quarantine tank, but I'm not sure what the likely culprit might be. Any thoughts?

He's about 3 1/2" long ex tail, and I've had him for about two years -- he was already about 2 1/2" when I got him.
