Could your DIY co2 mixture flow back into the tank? If it's a new tank i would suspect bacteria bloom but in your case it's unlikely. Pictures would help.
My whisker shrimp look exactly like ghost shrimp and have eaten two of my fish when they were sick. They are opportunists that will grab a fish if they can. Maybe you have a whisker shrimp.
