Bally's 40B

They do breath a little more rapid than many fish. Is he perched in a hole most of the time? Not sure about the upper lip and being wild caught (would be interested in seeing those references) but my guess would be a coral/nem sting. Since they are known to pick at rocks and eat detritus out of algae (many people think they are going for the algae), they will sometimes get a little close and get stung. Hope that's all it is and we don't break your pattern :).

He is in a hole 90% of the time. Before he was coming out of it to eat, I haven't seen him at all today. Fed some food soaked in fresh crushed garlic and he didn't even come out of where ever he is.
