Black Phantom Tetra sick -- HELP!!

Hello all. My son has a 5 gal. with 2 black phantom tetras. One of his fish has something funky going on. His face looks like he's been rammed into the side of the tank, and his gills look like they are wasting away. They are the only things in the tank besides a few snails and some great plants. The water specs are terriffic and they get weekly water changes. I've been doing fish for years and have never seen anything like it and I can't find any pics. Sound like anything y'all can diagnose?? I've added some aquarium salt, some melafix and some maracyn to the tank just to be safe but I've never had a sick fish short of the extremely old betta that I tried to save a few years back. I don't know what to do and I don't want to kill him. Any help would be great! Thanks.

Are you talking about injuries, or a growth on the fish? Anything it could have gotten caught in?
