Cabomba Dying!?

Why would my beautiful Cabomba be dying?? All the leaves are falling off and it's turning kind of brown-ish.
It's in a 5g.. I put a small pinch of freshwater aquarium salt with every water change. I have a 20watt bulb... no Co2 .. no fertilizers or anything like that.
What can I do to bring it back.. or is it too far gone... ???
Or what's causing it to die..

I agree adding salt to a small tank after every water change is a NO-NO. Do you change ALL the water? Salt does not evaporate. It remains in the water column and when you replace water and add salt you are actually making the tank more and more saline with every change.
