Can anyone ID this coral for me?

I got this coral for free , but i have no clue what it is, i stuck it on a rock in my tank but its not doing very well, can anyone help me ID it so i can figure out what i am doing wrong, it looks like a plate with polyp holes its brown/tan in color, i am pretty sure its a SSP and i was told it has yellow polyps that come out of it. Sorry about the picture quailty but i had to take it with my Iphone. ne1 have a idea? i was thinkin it might be a encrusting coral but i wasnt sure, and its not attached to anything...

Hard to say from just the pic, looks like it could be a species of Turbinaria. Does it secrete lots of mucus when its handled?
 
Right now i have the plate sticking straight up and down, with nothing touching it, it looks like it is slowly dieing from the bottom up and bleaching out, it has some mucus when i touch it, and i know i am doing something wrong because the polyps have never come out all the way yet >< thanks for the help!

How long have you had it?

I'd wonder why it was given to you free, maybe it was already unhealthy?

Turbinarias spp aren't as bothered about lighting as a lot of other corals are. They do adapt well to most kinds, mine started under standard tubes and I thought it was doing well there until I stuck it under halides!!!

They do however need a good flow. Mine sits right under a Koralia 4 powerhead and gets a good blasting. Yours looks slightly" sloped" in shape so you need to try and get the flatter side facing downwards so that any "bits" don't settle on it.

If it's at the top of the tank I'd move it down a bit so it can adjust to your lighting. Don't worry about target feeding it just cyclopeeze or similar when feeding your fish - I've never target fed mine yet!

When open it should look like a yellow version of this:

Not all turbinaria open up like that. What you have is Turbinaria peltata. The one that rindotheraven has looks more like like turbinaria reniformis which is more like an SPS coral in that its polyps are very small an will only extend a mm or so
 
You cant tell to well from this pic but my turbinaria reniformis (yellow, top of tank) has its polyps extended
