Is your tank exposed to sunlight? If it is, then algae growth might be a problem. If your tank just look generally cloudy, then you should do up to 90% water change for the first few days until the cloudiness go away, then you can cut back from there. Also, drain out your hang on filter, clean it and replace your filter cartridge.
 
I think the fish are dying ar acting strange after large water changes becaue they are accustomed to the worse water conditions (osmoregulation) and the fresh water actually shocks them. This is caused by OTS which is caused by Heavy Bio-load, infrequent and/or too small water changes. 12-20% weekly is not large enough water changes for a heavy bio-load. Dissolved organic compounds build up and make the water "thick" (OTS Old Tank Syndrome). This is not good for the fish and will stunt their growth and slowly kill them. However, they do adapt to the "thick" water. When you do a massive water change when this condition exists you shock the fish by "thinning" the water quicker than they can adjust. When you have a large bio-load and have been doing regular small water changes this will lead to OTS. To remedy this you need to change out a small percentage of the water (5% or so) DAILY for 2-3 weeks, then the water should be healthy enough to start doing weekly or more 50% changes, which is the minimum with a largely crowded tank.
