I wonder if it was a grossly dirty tank and they tried to clean it with bleach? Bleach + ammonia = deadly gas.

that sounds really odd, even if they did get an ammonia bleach combo, wouldn't the ammonia be so low that there would be no effect?

It also doesnt say what was living in the aquarium, for example if they are alergic to beestings a lionfish can be leathal, so can some types of coral or a carpet anemone.
