Dwarf frog disaster!

I've had 4 dward frogs for 2 weeks now in a 2.5g. They have eco-complete, 15w screw-in, 72-78F, some creeping charlies, 2 small mapani roots. This morning i found 3 of them on the carpet, and one dead in the tank. I managed to get 2 of the live ones in a jar with clean water, and they're still alive. I have no idea what happened, the water must have been toxic to the frogs for them to all jump out in the same night. The water parameters are as follows:

pH: 7.5
NH3: 0
NO2: 0
NO3: 12.5
kH: 4
gH: 6
PO4: 0.25

Non of the parameters have changed at all in the past few days, so i'm totally stumped as to what could have happened...I'm extremely upset...this sucks...

The only thing i can think of, is if a test kit is bad. I haven't added anything into the tank for a while, besides frog food.
