Female Swordtail problem

I have a tank full of pineapple Swords and a few other fish. The swords are all from the same spawn and are about 3 years old. I have been losing a female swordtail every month or so (the females outnumber the males.) I had assumed that the fish were just dying of old age, since it was happening so infrequently, and every other fish in the tank looked healthy.

Well, today I see a female Swordtail on her side on the top of the tank:

When I zoom in on what looks like the prolapse, I see this:

Is this a delivery gone wrong? Should I grab this mess and try and pull it off the fish?

I have two other female Swordtails in the tank with clamped fins now. All the males, the angelfish and the zebra danios all look just fine.
 
Andy, how'd you make out with this?

FWIW, I've never had a livebearer live longer than 2.5 to 3yrs. I'm sure absolute ideal breeding and keeping, they'll live longer like anything else.
