HELP!!! My Pennywort is dieing!!!! It may already have died...

Its sort of like an EXTREME form of decay... my aponogeton capuronii is starting to die off and it just happens to be right next to the pennywort. My other penmywort on the other side of the tank seems unaffected though. The aponogeton is/was THRIVING In this setup. 2-3 leaves a week no co2. Over 16+" high and the stems are just breaking off. Little threads like sowing threads are connected to the base of the where the stems used to be. It turns completely translucent breaks off the main plant and floats and proceeds to infect the nearby plants.
