help needed with cleaner wrasse

How long have you had it? Is it feeding?

Cleaner wrasses seldom thrive in an aquarium--there just are not enough parasites to support them. They usually starve--and yours, sadly, is displaying many of the symptoms of starvation.
 
Regretfully, there isn't much you can do. Cleaners need lots and lots of food--enticing it into eating, possibly with some live brine, might work out. BOL!
 
This is one of the reasons that Cleaner Wrasse should not be offered for sale. It must be either left in the ocean or kept with many parasite-infested fishes. At this point, the best thing you can do is to make it accept substitute food, but from what you said in the post, this is highly unlikely to happen. Is there any way you can return it to LFS?
