If you've already seen an ortho, I would imagine that you've had an MRI. If you've taken nothing this far, take 600mg Ibuprofen (Advil) 4x a day. You'll probably see a difference in about a week. Don't take this dose for more than a few weeks without taking to your primary MD, as Ibuprofen can cause renal problems if taken long term. If that doesn't work, get a referral to another ortho. It never hurts to get a second opinion.

For the love of god... please don't start taking meds based on the recomendation of someone on a fish forum that you don't even know. If you're not happy with the treatment you received, or if you haven't noticed sustained improvement go back to the doctor. Odds are they have more options that they didn't offer because they thought the first thing would work. This is not the place to get a diagnosis or medical advice. There are great pain management programs out there. Go make them find you one.

Injury to the rotator cuff is nothing to mess around with or to try to cover up with pain meds. Doing so could make matters worse. You need to go back to your orthopedist, tell him/her what you didn't like about the PT or why you think it didn't work, and find out what your options are. Ask the doc to explain the problem to you again — and keep asking questions until you are completely clear about what is going on with the joint, what you can do to prevent further injury, and what can be done to repair the damage that's already occurred. If your doctor cannot/will not explain things in a way that makes sense to you, find another doctor. Some are better than others at that sort of thing — but it's your job to tell the doctor you need more information/help.

Look, lots of us have no insurance and little money, so are stuck with "Dr. Google" for diagnostics and care suggestions. You are not in that position, so take advantage of it. A consult with the doc on a case he's seen you for before is not going to be expensive. And for the love of mercy stop abusing the joint with straining exercise, or any exertion you can avoid, until you have a clear understanding of what's going on in there. Yes it's possible that the PT will cause things to hurt worse before they get better, but know why it's hurting and what the end goal is.

The blunt truth is--as with your foot situation--you have the body you have, were born with. This may not be the body you wish you had, or one like "everybody else" has. (every other body has its own set of quirks too of course, you just don't see them cuz you're not in that body.) But it's what you've got and you have to learn to work with it, not constantly be trying to make it do things like "everybody else." I think once you stop fighting it, it may stop fighting back.

I couldn't agree more. I was simply trying to offer a stop-gap measure 'til he could see his real doc. This forum, while full of awesome aquatic info, is certainly not the place to get major medical advice :)
 
At this point you probably need a rheumatologist and an MRI.
It sounds like you started with Crepitance and let it get worse.

Crepitance means a crackling or grating sound- and it is most often used to refer to bones rubbing against one another.
