losing fish, can't figure out why, suggestions?

fish only set up for about 3yrs... In the past week I have lost a foxface lo (3 yrs in tank) and lawnmower blenny (6 months in tank) have a coral beauty and yellow tang left in tank. No signs of disease that I can see. Just took readings they are:

Ph: 8.2
Trite: 0
Ammonia: 0
Trate: 20
Salt: 1.023

using only distilled water for water changes and change 15% water about every two-three weeks

Any ideas?

The losses may not be related.
Three years for the Foxface is not too bad, considering you kept it in a tank which some consider small for this species. They need lots of algae in the diet. If there was/is a problem, the YT would likely be the first to indicate it. Again, make sure daily green foods are on the menu.
