Nutrient def/overdose ID help needed

I have hygro polysperma "rosanervig", which is supposed to be a fast grower showing signs of trouble. It is growing, but some of the stems, where they are growing out of the substrate, are dark and mushy, and eventually die.
I have diy co2, flourite substrate. I was dosing too much iron and have now revised my fert dosing. Has anyone else seen this problem before?

Can you post the needed information regarding your tank, such as, No3, K, Po4, and traces? What's the tank size, and wpg? How much Co2 are you injecting?

This info would be of great help to us in order to help you. If you can, post a picture of the plant in question.

What is your lighting? If the lighting is too low, the lower stem dies off allowing the plant to float and be closer to the light. Not that the plant is thinking about this...just happens.
Otherwise, the soil/substrate is too "something" (acidic?) and causing the stem to die.
