Paradise Fish In Trouble

The problem first came to light about 5 weeks ago now, my paradise has 2 white spots on his pectrol fin which are quite large (larger than grains of salt).My first thought was it was Ich having read a lot of your threads about that desease. So i upped the tempreture like everyone has sugested, so the Ict would hatch and i then could treat it in it's free swimming state.Well my temp was running to begin with at 74degree so i upped to 80 and to cut a long story short my paradise still has the spots. I had already before i upped the temp had treated with the right med plus salt and since the temp change i have done this procedure a couple of times.
I just have no idea on what it could be the fish looks ok he is feeding well, the only other thing now i have noticed is that he has in the last couple of days gained an open sore just on the tail. I notice yesterday that his tail now has split totally back, im feeling really helpless and i dont wont to lose this fish having brought him on and he is believe it or not totally tame to me he knows his mum !!!!!!!!!!. So please help and give me some idea on what to do, thank you for reading this.
 
Paradise and gouramies are both prone to a rather nasty bacterial infection--usually doesn't show up until you've had the fish for 3-4 months. Remove to a quarantine and treat with a broad spectrum anti-biotic, changing 50% of the water daily. It requires aggressive treatment--melafix and the various 'natural' treatments are ineffective.
