Problems with Ottos

For the last couple of days I have noticed that my 3 Ottos have been swimming upside down on the surface of the tank. This morning when I got up one of them was dead. Any ideas on why they are acting like this? Also the tank is a 54 corner bowfront and all water parameters check out ok. All other fish seem good and healthy. They are all eating and breathing normally. Filtration is 2 HOB bio wheel filters with media changed once a month and weekly water change of about 50% and moderatly planted with live plants. I dose Flourish and Flourish Excel at the recommended dose. Lighting is 2 30" standard output. Thanks for any input.
 
How long have the oto's been in the tank? Do they have plump round bellies or are the skinny? Do they have full heads or is the area behind their eys sunken at all?
 
The Ottos have been in there about 2 months. They dont look skinny but they dont look fat either. There heads look fine. There heads dont look deformed or sunk in. Another thing I never have to clean algea off the front glass so im not sure if that might be an indication that they arent getting enough food.
 
when was the last time you changed the filter media? what exactly are your ammonia/nitrie/and nitrate? there is really no need to replace filter media until it is falling apart. if it gets clogged swish it in a bucket of old tank water to get the gunk off.
