Rotting Java Fern

My Java Fern has been growing well for almost 2 years. Now in the past 1-2 months most of the leaves are rotting at the base. It is a different look than the leaves that deteriorate due to a sprouting plantlet at the end. All of the old leaves have these expanding holes only at the bases that is causing the leave to completely rot off the base. Also, all the small, newer sections of the plant seem to have stopped growing.

All my other plants seem to be doing pretty well.
I dose potassium nitrate to 5-10ppm, fleet's enema to 1 ppm, Fluroish Iron, and I add plenty of KCl. I also use Fluorish Comphrehensive.

My pH is usually about 7.0 and the kH 2-3.

Any ideas?
 
I have a question: What is Flourish Comprehensive? I can't find it on the Seachem website. Do you mean straight Flourish?
