Just a hunch, but I'm wondering if the condition is stress related. Do you remove your fish to clean the tank? Do they get stressed out during water changes? Like a kid with a bad case of zits, stress may bring a chronic underlying problem to the surface. Odd analogy, I know, but it's the first thing that came to mind.
