ammonia is at 0
dont know about nitrates and nitrites,
ph 6.9
i do 50 percent water changes weekly

Quick guess would be a fungal infection. Try a commercial medication. E.M. Tablets worked nice for one fish I had that had fungus on the mouth.

would it be wise to take him out and put him into a QT tank
