Unknown Oscar sickness

Does anyone know of anything that can cause sunken in eyes on an Oscar. I’ve been gone for two days and got a call that he was laying at the bottom of the tank and has a large hole on his head with black around the edges. It appeared suddenly so I don’t know if it’s from his usual random thrashing or HIH. Other than that his eyes are sunken further into his head as well and I cannot find anything about this. I started doing a water change and about 15 minutes into adding new water he is swimming erratically and sideways and then sinking to the bottom. I don’t notice a bloated swim bladder so I’m not sure what’s causing the swimming.
All parameters were normal besides nitrate which usually spikes until I do a water change. Took out the carbon filter. Any help appreciated

He has also started swimming in place into the glass
 
Sorry to hear about this, but if you're still here, we need lots more info. What size tank is the oscar in? Tank mates? How often have you done water changes and how much water do you change each time? What type of foods and feeding schedule?

I had to put down my 12yr/old oscar this past summer. He laid on the bottom of the tank and wouldn't take food for months.
