what happens if a fish eats carbon?

I would think if a fish ate some carbon it would spit it out unless it got caught in its mouth or throat, or maybe gills. It might happen. Very small bits should pass through the digestive tract & out the vent. I've never had any of this happen...in 40 years of fish keeping, but many things are possible in freak accidents.
 
Why do you need carbon anyway? Probably you don't..... but I know this a longer discussion.
